NOTES:
1. ADDRESS = IC(BPT) initiated the Hold state.
ADDRESS = IC(Hold) - 2, if HOLD = LOW initiated the Hold state (unless changed by the monitor system).
2. Other output states: DMAK HIGH.
3. Other required input states: DMAR = HIGH and RESET = LOW.
